Performs advanced, specialized work to assist in the organization and implementation of various projects and programs. Work is performed under minimal supervision within established policies with latitude for independent judgment. Works on special projects as assigned. May substitute for supervisor during his/her absence.

Required to maintain electronic calendar current by making daily changes. Calendar should reflect patient related meeting information and activities, internal and external functions, meetings, and events.
Required to communicate and review changes to the Project Coordinator as directed.
Demonstrates a professional attitude and effective communication skills. Provide information to navigate patients to better health.
Refers patients to appropriate agencies and follows appropriate referral procedures. Keep records of referrals and subsequent follow-up in patient record. Assists in navigating patients for health care services through referrals.
Maintains data accurately in appropriate systems such as EMR; review charts and obtain data as assigned.
Responsible for tracking assessment forms, clinical measures, mileage, referrals, dismissals and weekly visits on a working matrix to be submitted to Project Coordinator as directed.
Bilingual in Spanish and English, must be able to communicate effectively (verbally & written) to meet the program measures and patient needs
Provide instructional classes and presentations.
Travel for continuing education (CEU).
Education and Experience:
High school diploma or the equivalent.
Certified Community Health Worker(CHW) or be enrolled in a CHW program at the time of hire.
Six years of experience in related field. Additional education may substitute for experience on a year for year basis.
Personal transportation required to complete major essential functions of the position.
Upload CHW certification or proof of enrollment in a CHW program.
Letters of recommendation must be dated within 12 months.
Completion of a Community Health Worker (CHW) program with 2 plus years experience working in a CHW position with patient related experience.
Outreach experience to include networking, developing a community resource pool for patient referrals and networking.
Experience conducting patient referrals.
Working knowledge and understanding of HIPAA.
Experience in interviewing and conducting patient visits.
Experience in data collection, reviewing assessment tools, and data entry in an EMR system or other related electronic application.
Experience working with patient information within an EMR system and data management.
Experience maintaining a daily electronic calendar.
Experience generating reports related to the program directives.
Experience working with a diverse population, faculty, and staff.
Experience tracking work related mileage and completing required documentation.
Demonstrate an understanding of time management required for accountability of time.
